Federal Tax Information
From January 2 through April 30, hard copy and/or reproducible copies of many federal tax forms and instruction booklets are available at the Urbandale Library. We currently have some free copies of the 1040, 1040EZ, 1040A, 1096, 1099, W-2, and W-3 Federal tax forms available at the entrance of the library. Federal reproducible tax forms are kept in a binder at the Reference Desk for one year.

State of Iowa Tax Information
Please note: The library will no longer have paper copies of Iowa tax forms or instruction booklets. With the continued growth of electronic filing, the Iowa Department of Revenue (IDR) is no longer providing paper income tax forms and instruction booklets to libraries beginning with tax year 2011. This change is part of the IDR’s focus on creating efficiencies through the increased use of technology. Please go to http://www.iowa.gov/tax/forms/obtainforms.html for more information and instructions on how to obtain a paper Iowa tax form and instruction booklet. The library is also happy to assist anyone in locating a form online, please stop by the reference desk for assistance.
